Students must demonstrate eligibility for a Children First Scholarship every three years. Children First uses the original date of eligibility you provided by a previous school district through an IEP process, a Multidisciplinary Team Evaluation (MDT), or an Assessment Team Meeting Record (ATMR) associated with the Carson Smith Scholarship program.
If you have received a notification that your child is due for a re-evaluation, this may be performed in one of the following ways:
1. If your child is homeschooled, request your local school district to perform a re-evaluation. Click here for a form that can be used to make this request.
2. If your child is enrolled in a participating private school, request the school district in which the private school is located to perform a re-evaluation. Click here for a form you may use to make this request. Or ask your school if they are able to conduct a re-evaluation. Click here for a form that can be used to complete the process.
3. If the above methods do not work for your family, Children First may be able to assist you in finding an appropriate alternative.
